Help to Ensure Safety, Security and Quality Service in Your Community
As a community member, you may voice your complaints against existing businesses in your community. You can report any issues you find to be problematic including overcrowding, alcohol or tobacco sales to minors, prostitution, sale of illegal drugs and many other issues.

After downloading, print the form, complete it, and send it to the mailing address located on the form. You may also fax the complaint to the number listed at the bottom of the form. Your complaint will be handled by the City of Chicago Department of Business Affairs and Consumer Protection.
